    Dr. Carlos A. Plazas


    Dr. Carlos A. Plazas the President and CEO of Forward, P.C. is a Licensed Clinical Psychologist in the States of Illinois and Florida. He was born in Colombia, S.A., obtained a doctoral degree in Pedagogy, Philosophy and Letters at University Javeriana Bogota, Colombia; immigrated to the U.S.A. in 1967: studied at Loyola University, Chicago and obtained a Master and Doctoral Degree in Psychology and Measurements. He was appointed Executive Director of Edgewater-Uptown Mental Health Center, which at that time served 1300 patients per month.

    Dr. Plazas founded St. Augustine College, which was approved by the Illinois Board of Higher Education and became the first bilingual institution of higher education in the Midwest. Throughout its 34 years of existence, St. Augustine College has made higher education accessible to more than 30,000 students, mostly of Hispanic and minority descent.

    Dr. Plazas incorporated his private practice in the State of Illinois to continue providing mental health and psychological services. His firm is called Forward, P.C. This title typifies Dr. Plaza’s treatment philosophy: “Always moving forward and making progress.” Recently Dr. Plazas received the two new licenses of the State of Illinois to evaluate and treat Sex Offenders.

    Dr. Blanca R. De la Torre

    Board Corp. Secretary

    Dr. Blanca R de la Torre Plazas is an Illinois and Nationally Certified School Psychologist with bilingual (English/ Spanish) endorsement. She worked 20 years for the Chicago Public Schools serving children from two and a half years to high school age. She also helped parents and school personnel make important educational decisions. For 10 years, Dr. Blanca Plazas was the Vice-President of ST. Augustine College for Research and led the Institution to obtain the accreditation of its educational program by the North Central Association of Colleges and Universities. She has been the Corporation Secretary of Forward, PC since the inception of this organization over 25 years ago.

    Dr. Blanca Plazas was born in Cuba; came to the U.S.A. in 1964; obtained a Bachelor of Music Degree from DePaul University (Chicago); a Master of Science in Education Degree (Educational Therapy and Special Education with a Teaching Certificate in Learning Disabilities) from National Louis University (Evanston, IL); a Doctor of Education Degree (Educational Psychology) from Loyola University (Chicago). Likewise from Loyola University, Dr. Blanca Plazas obtained a Certificate of Advanced Studies in Organization Development.

    Devora Diaz, B.S.W.

    Program Director

    Devora Diaz has been serving as a Family Counselor and Program Director at Forward, P.C. for more than six years. She has been committed to provide school youth and their parents with counseling services in coordination with school social workers and counselors.

    Ms. Diaz obtained her Bachelor’s in Social Work in St. Augustine College in Chicago, IL. She is also currently purusing her Master’s Degree in Social Work –Gerontology. Ms. Diaz has provided trustworthy professional counseling services to a number of clients within the frame of honesty and community cooperation.

    Lizette Paz, B.S.

    Accountant Officer

    Ms. Lizette Paz joined the Forward, P.C. staff in 2014, to assume her role as Accountant Officer and Administrative Assistant. Ms. Paz is in charge of handling all the accounting responsibilities including but not limited to; billing, budget administration, and assisting with case management.

    In 2010 Ms. Paz received her Bachelor’s Degree in Business Administration with a Concentration in Accounting at Robert Morris University. She is currently enrolled at University of Phoenix and diligently working to obtain her Masters of Science in Accounting. At her leisure she enjoys spending time with loved ones and aspires to become a Certified Public Accountant.
